1. REGULAR MEMBER A regular member, age 18 to 65, is the one who complied with all the requirements for membership and is entitled to the rights and privileges of being a regular member. He/She patronizes cooperative products and services. 
2. ASSOCIATE MEMBER An associate member is the one who patronizes cooperative products and services, and has no right to vote nor be voted upon. He/She is entitled only to such rights and privileges provided in the Cooperative by-laws. 

• Attendance to PMES and Ownership Seminar
•  Completely filled-out application form
•  Birth Certificate/ Gov’t issued ID with Date of Birth/ Marriage Contract (if applicable)
• Two (2) 2×2 colored ID picture (recent shot in white background)
•  Taxpayer’s Identification No. (TIN)
• Medical Results (for 18-50 years old) or Waiver (51 to 65 years old or those who did not submit medical result Complete Blood Count ( CBC ) validity from the date of Lab test is 1 month Urinalysis – validity from the date of Lab test is 1 month Chest X-ray result – validity from the date of Lab test is 6 months –      
•  Capital Share: Minimum – Php3,000.00  (to complete in 2 years)
             –  Paid-up – at least 20% of the subscribed capital (minimum Php600.00)
• Membership Fee- Php150.00 o $50 for OFW investor

• Complete filled-out application form

• Valid ID
• Capital Share: Minimum – PHP3,000.00 (to complete in 2 years)
